To get Nirvana
Go to the monster arena in the Calm Lands. Once you capture at least one of each type of monster in the Lands, you'll be able to use the facilites of the arena and a treasure chest will fall from the sky containing Yuna's Nirvana. You can then open the chest with the Celestial Mirror (more details on the monster arena here).
To get first power up item: Moon Crest
The Moon Crest can be obtained very early in the game. It's in a treasure chest at the beach of Besaid Island. Go to the extreme left of the beach where you first met Wakka, then swim out a bit and continue left. You'll soon come across a little hidden inlet with the treasure chest.
To get second power up item: Moon Sigil
You'll get this the first time you defeat the Magus Sisters at the Remiem Temple (more details on the aeon battles here).
