Eyes Bead chain, dumbbell, or Spirit River Eyes-Balz - material and size depends on the amount of weight you want to add. Example has Eye-Balz.

Tail 1-inch long straight-cut chartreuse bunny fur.  Tie middle of strip at the bend of the hook, but don't trim off the leftover. The remainder will be brought over the top after the body is wrapped.
Body Chartreuse polar chenille wrapped forward and stroked rearward. Bring the remainder of the bunny strip over the top of this and tie off behind the eyes.

This fly can be found in Fly-Fishing for Pacific Salmon II, by Les Johnson and Bruce Ferguson. It is mentioned there as a fly for Coho, thought the version shown is purple. The Chum Salmon run this year has been a disappointment, but this one has at least taken a couple.
