The pattern for this fly, originated by Walt Johnson, came from the fine book by Dick Stewart and Farrow Allen, Flies for Steelhead.  They write that this fly was accidentally originated when Dave McNeese confused the dressing for a Surgeon General and tied several dozen for his shop. He put them out for sale anyway and the fly now has a proven record of success.
