Jake grew up intending to live and work near his family, but always with the intent of becoming a doctor. Upon graduating from medical school, Jake fulfilled his dream and opened a practice just a few miles away from his boyhood home. Despite working long hours, Jake fell in love with his receptionist, Carolyn, and the two were married, producing two beautiful little girls, Julia and Maria, both blond-haired like their mother, Julia having a compassion for all living things, and Maria tending to be a bit reticent like her father, but curious as to how everything in the world worked and moved and breathed. Just when it seemed that things were as perfect as they could get, Jake was working late the evening of his birthday. Rather than make Jake eat a warmed up dinner and not get the chance to see his two darling girls before they went to bed, Carolyn decided to take them down to where Jake was working, birthday cake in hand, and presents dangling from the hands of the little girls. As they crossed the street in front of the medical complex, a hovercraft veered out of control, crashing into them and killing them instantly. Jake barely remembered the first few years after their deaths, and barely kept his medical practice going. When it became apparent that trying to live in his house without his family was tearing him apart, Jake took a drastic step and joined Starfleet, hoping that the further away he got from Earth, the less it would hurt. After two years spent in Starfleet, Jake realized finally that the hurt was never going to go away. To anyone that didn't know what had happened, Jake Serren presented as an amiable doctor flustered by nothing. He was the same all the time, whether during battle, or a routine day of sick bay duty. It was not until the arrival of a new CMO to Jake's first posting on board USS Pacifica that Jake ever revealed what had happened. Ilya Karasov, the new CMO, had been a confidential listener from the very start, and he hadn't told Jake what he should do to pull himself out of the rut. Instead, he said nothing, offered no advice, but was somehow always around whenever Jake was having a low point. It was not until Ilya was badly beaten to near death that Jake realized that there were people who mattered to him after all. In his desperate attempt to help Ilya regain his speech again, Jake enlisted the aid of Science Officer M'Dray Kelly. Her compassion for Ilya, who was a stranger to her at the time, attracted Jake to her. Since that time, Jake has become a bit more animated, though still retreats into periods of silence every once in awhile. When the attack on the USS Pacifica came about, Jake's lifepod was among those rescued by the Federation. He spent time on Starbase 32 as its chief medical officer until reassignment to the USS Iliad, only to find that his friend and former CMO had been promoted to captain of that ship. On the eve before the launch, the captain and first officer were indicted for causing the attack on the Pacifica. Jake, knowing Ilya Karasov perhaps better than any other man, instantly offered his testimony, but to no avail. When it became apparent that the two would be indicted and court-martialed, Jake offered his help in a plot to steal the Iliad and rescue the Pacifica crew still imprisoned at Satarra. When he, along with the crews of the Lexington and Iliad, came back with 5,000 freed prisoners, the planetary governments of all the races involved sent words of congratulations and offers to join the Federation. In the midst of all that, the Federation could hardly brand as traitors those who had done so much to further a peaceful alliance in the galaxy. Captain Karasov, along with Commander Hunter and the others who had helped to steal the ship, were given conditional pardons, and their ranks were returned to them. Jake now enjoys the status of CMO aboard the Iliad. He is still reticent at times, but speaks his mind more readily, and has kept his stubbornness and dry sense of humor.
