In order to get your free report, please follow the following directions:

Fill in the following form and then press the "Submit" button.
When I receive your report request, I will e-mail you with an order number. Write this number on your dictation media.
Send the dictation media with order number to:
L.M.S. Transcription Service, 113 Laverne Lane, Walhalla, S.C. 29691.

PLEASE NOTE:

Phone-in dictation is not available at this time. It should be available soon. Please check this website for updates on this service.
You will be notified by e-mail when I have received your dictation media. From that moment, you will receive your document via my standard e-mail service: delivery in three (3) business days via encrypted e-mail (you will be provided with a password to view the document)
