• Membership eligibility requirements: Undergraduates must have completed a minimum of 12 credit hours in history courses, with a GPA of 3.1 or better for all history classes, and a cumulative GPA of 3.0 or better. Graduate students must have completed a minimum of one third of their residence requirements for the Master's degree, with a GPA of 3.5 or better for all courses taken for graduate credit, and no grades of ‘fail’ (or equivalent) on graduate record. 
• Complete the form below, including your name, address, phone number and email, as well as all history classes you have taken at UF, grades received in them, and your cumulative UF GPA.
• Attach a check for your membership application fee of $50 at the top of this form. $30.00 of this fee will go to your International History Honor Society lifetime membership, which includes a year’s subscription to The Historian. The remaining $20.00 will be used for Gamma Eta Chapter resource funding. Checks should be made out to Phi Alpha Theta.
• Sign the Honor Statement at the bottom of this form. It is important that the officers of Phi Alpha Theta can put their trust in your honesty. If any information on this application is found to be falsified, you will have breached the Honor Statement and will be subject to dismissal. Academic dishonesty is not tolerated.
