The Doctor's Famous Ordering Method

To order a delicious and world famous Dr. Katcher pizza simply fill out this special order form listing the toppings you want in the order you want them placed on the pizza (remember the price and amount of topping varies with the order in which they are placed on the pizza -- see pricing), whether you want a whole pizza or just a slice, and how much you belive the pizza will cost you. Please also enclose a self addressed stamped envelope for the pizza to be mailed to you.
