RATES & SERVICES

Base Rate

$2650 to $3050 per Month, including room, board and minimal services

 

 

 

 

SERVICES INCLUDED IN THE LOW BASE RATE

* Initial health care assessment by staff registered nurse to ensure that physical, social, and emotional needs are met

* 24 hour supervision with trained medical staff

* Medication distribution

* Minimal assistance with daily cares

* Laundry and Linen service

* Three meals a day plus snacks

* Recreation, exercise, & spiritual activities

* Housekeeping and maintenance

* Transportation for medical appointments on weekdays in Hutchinson

* Transportation provided in Cosmos facility for medical appointments in Litchfield & Hutchinson

* Many other services available at additional charges such as: physical therapy, incontinent cares, assistance with oxygen, assistance with ambulation, feeding, total cares with dressing and daily needs, etc

Two locations providing assisted living plus services. the only facilities in the area that provide either minimum services or up to and including total cares.

Both facilities are licensed by the Minnesota Department of Health as a Housing with Services Facility and the Minnesota Department of Human Services as an Assisted Living Home Care Provider. We offer a variety of services ranging from minimal services up to and including total cares. Each facility has private resident rooms, welcoming sitting areas, and attractive outdoor sitting areas.
