I had just finished trying - without success - to get good paresthesia coverage of the left SI joint using a conventional SCS placement. The patient had a "dead spot", i.e., we could get leg and abdomen but not low back. I decided to try running a lead up the sacral sulcus. I placed a 30 degree bend in the needle and ran it along the posterior surface of the left lateral sacral border.
