Telemedicine (Tele = Distance ~ Medicine) is concept of providing health care to the patient instead of him traveling to unknown and strange places at enormous cost.
Imagine a villager in Bihar with an ulcer in his cheek who urgently needs an opinion whether the disease he has is cancer or not. In case it is cancer, he has to come to a cancer surgeon and get operated followed by other therapies, which if given early can let him lead a normal life. In case he does not have cancer, he need not spend any money for traveling etc.
With current technology of instant communication and internet, it is possible to transport the patient virtually to the specialist - in the form of his reports etc., get an opinion at a small fraction of the cost of a personal visit. And also it will be done in no time. In the above example, The patient's Biopsy slides only need to be transmitted through the to a specialist pathologist anywhere, but the specialist can if required manipulate the position of the slide to look for cancer cells in a specific area, as if he had the slide in his own hand.
Why do we need computerized patient record keeping
For a busy doctor, most of his time in his OPD is spent in patient care - taking, the history, examining, operating, giving an opinion, and administering therapy. When the prescription is given, the patient is asked to bring his reports and prescription slip for his next visit. To allow for the frequent problem of the patient losing the slip or reports or not bringing it on subsequent visits, a copy is supposed to be kept with the physician - (recent laws have made it mandatory for the doctor to keep this copy).
Most big hospitals, however keep a meticulous record of the minute to minute and daily interaction with the admitted patient. In both cases, the time spent on writing the records is an enormous drain on the physician and his /her staff. So much so that most physicians don't keep as extensive records as they should.
If one has really tried to imagine the time required to manually writing one long prescription, AND filling in the history, AND name, age sex in each investigation form. and report AND reasons for the investigation AND operation notes, one can also imagine the sheer drudgery of it. Some of it will always be required like prescriptions. Even after that certificates and bills will be demanded occasionally. But most important, if the patient does not bring his previous record the next time, the entire exercise of history taking is repeated etc.
Now imagine a system of keeping records so that the time spent on writing the record is very short, even less than just manually writing one long prescription. In this time the record is not only written but available for all subsequent visits too without even a prolonged search in the record room.

- Level ONE OF THE SOLUTION
IT caters to the needs of the General Practitioners having independent clinics or small nursing homes and helps in the maintenance of basic records of patients as well as creates the Telemedicine consultation document for referral to the Telemedicine center. It may be noted here that even this level can interact directly with level three special if the diagnosis or opinion needs to be made only through the history, physical checkup or simple investigations - which is true 80 % of the time

- Level TWO OF THE SOLUTION
The Telemedicine center shall receive the document from the referring physician and transmit to the consultant. Further requirements by the consultant like CT scan, MRI, etc. shall be organized at this level. The report from the consultant shall be remitted back by this center to the referring physician

- Level THREE OF THE SOLUTION
The Consultants, whose opinion is being sought. Institutions like AIIMS have to be persuaded to be in this level.

Consultant level Software and hardware (Optional - They
can easily be provided a hard copy and their views noted through a secretarial
service provided by the Telemedicine project vendor.)
Premier Institutions may be provided equipment by us at our cost, that may be realized through the payment made by clients for level two and the Institution shall be bound to provide the services.
