This sheet has been great for many things: It tells me a little bit about my students, and since it asks for a parent signature it's one of my first contacts. Also, on the back is a form with spaces for dates and comments. I keep these sheets all year, and keep track of disciplinary action and all parent contacts. It's a great way to keep track of what parents and students have said, and what I've said to them.
