The page not appearing at all can be caused by a few things:
a  Printer page size too small
b  Board too large, something out of bounds or something else weird
c   Delete .ppc and start a new one.
d   Press the 'show entire document button'

While it is a handy unit the print preview has some pitfalls.
1. Many refreshes are required to ensure you get the display up properly.
   Similar to the zillion presses of the end button required in pcb.
2. Sometimes a simple press of the 'show entire document' button is
required.
   Then it magically appears. Great huh.
3. Why can't it default to 'whole board on page' and a minimum of layers.
    The first time takes forever to appear. I mean forever.
4. Cannot easily select the pcb file you wish to use, you have to get it to
prompt for it.
5. Selecting/deselecting layers is a masterpiece of excessive mouse clicks.
