I very recently went to see Soil (who by the way, are fantastic live) with some friends, and we saw perhaps some of the most unlucky stage divers ever. It really was quite amusing.
The first notably one I saw was actually quite aerobic in his poor diving-ness. He climbed onto the rail over a thin section of (rather short) people moshing. He jumped, hit about one pair of arms ready to receive him, and rolled over to hit his back on the floor:

The second (who might have been the same guy, I forget) wasn't so aerobic, but could have been more painfull. This time he jumped, landed, rotated legs upwards, then fell:

This was just the general trend of the evening. One little mini-mosher jumped onto a small gathering of other mini-moshers, who, being unable to support any weight whatsoever, all collapsed:

Well, despite failing efforts like these, crowd-surfing is a long-lived tradition, and unlucky crowd-surfers; we salute you!
