The crew were performing a non-precision instrument approach to Pensacola Runway 25,
when the Boeing suddenly struck the water of Escambia Bay, 3nm short of the runway. The
aircraft came to rest in 12 ft. of water. The weather at the time of the accident was 400ft
overcast, 4 mls visibility in fog and haze, wind 190deg/7kts. PROBABLE CAUSE: Failure
of the flightcrew to monitor descent rate and altitude and the failure of the 1st officer to
provide the captain with the required altitude and approach performance call-outs. The crew
had a.o. also turned off the GPWS and failed to configure the aircraft properly and in a
timely manner for the approach. |
