INDEPENDENT CONTRACTOR AGREEMENT

The above parties agree to the following terms:
Independent Contractor agrees to provide psychic counseling services to network
callers.
Independent Contractor understands that logging off the system, for the day,
with less than a 14 minute average may result in a suspension of their extension.
COSMIC CONNECTION is not responsible for any lost wages as a result of the Independent
Contractor being suspended.
COSMIC CONNECTION OFFERS BOTH BIMONTHLY AND WEEKLY PAY, PLEASE READ CAREFULLY AS DIFFERENT RATES APPLY. PLEASE INITIAL THE PLAN THAT BEST MEETS YOUR NEEDS AND /OR GOALS.
Pay periods run from the 1st to the 15th and the 16th to the 30th/31st. Your
first check will take approximately four and one half(41/2) weeks. Subsequent
checks will mailed out twice a month, the
5th and the 20th.
________BIMONTHLY PLAN
Independent Contractor will be compensated as follows:
0-599 minutes per week @ .15cents per minute ($9.00/HR)
600-1399 minutes per week @ .20 cents per minute ($12.00/HR)
1400 + minutes per week @ .22 cents per minute($13.20/HR)
1400+ minutes per week with a 20 minute call average @.24 cents per minute($14.40/HR)
_________WEEKLY PLAN
WEEKLY PAY is an advance against the minutes worked in a particular pay
period. A psychic is eligible for a weekly check if the psychic completes 500
talk time minutes in a particular week. If psychic does not complete 500 talk
time minutes in a week the psychic will be paid at the regular bimonthly
payroll time.
THIS MEANS THAT YOU WILL RECEIVE A WEEKLY CHECK FOR EVERY WEEK THAT YOU
COMPLETE 500 MINUTES, BUT NOT FOR THE WEEKS IN WHICH YOU COMPLETE LESS THAN
500 MINUTES.
*Cosmic Connection reserves the right to move any reader from weekly pay to
bimonthly pay.
Independent Contractor will be compensated as follows:
0-599 minutes per week @ .15cents per minute ($9.00)
600+ minutes per week @ .20 cents per minute ($12.00/HR)
The Independent Contractor must take a call on the 1st or the 2nd and the 16th
or the 17th to receive the above rates. If no call is taken on a designated
day, then compensation will be five(5) cents less for the corresponding time
period. This agreement subject to change as per PRN condition or policy revisions.
PLEASE READ THE FOLLOWING CAREFULLY AND PLACE YOUR INITIALS IN THE SPACE
PROVIDED INDICATING THAT YOU UNDERSTAND FULLY AND ARE IN AGREEMENT WITH OUR
TERMS.

IS GUILTY OF ANY OR ALL OF THE FOLLOWING:

***PROMISING CLIENT FREE CALLS/”NO CHARGE FOR THIS CALL”
***SOLICITING CLIENTS BUSINESS FOR CONTRACTORS PERSONAL BUSINESS
***ANYONE WORKING CONTRACTORS LINE OTHER THAN CONTRACTOR HIMSELF/HERSELF.

fully understand my responsibilities. I release COSMIC CONNECTION from any liability
and or obligation or withholding any FICA, Federal, State, or Local taxes. I
fully understand that all monies paid to me by COSMIC CONNECTION will be reported
for Income Tax purposes on IRS form 1099.
