The boys are rolling in the dough, their calling card scam is turning out to be quite profitable. However, we catch a sidelong glance from Tony as Pussy describes all the working details to Furio. Tony even surprises Carmela with a sable coat, which leads to the second time we see them be intimate with each other. Maybe Tony can be faithful after all.
Tony starts having strange dreams, and wakes up suffering from what is assumed to be food poisoning. In the first dream, he sets himself on fire, so that everyone doesn't have to witness his suffering from an unmentioned disease that leaves him until September 5th to live. In another dream, Silvio announces to Tony, "Our true enemy has yet to reveal himself." The next dream has Tony playing cards with Paulie, then pulling a gun and shooting him. This is followed by a therapy session where Melfi's persona becomes mixed up with Annalisa's. Then there's the truly bizarre sequence where Tony gets into a tiny car with Adriana and Christopher, then Furio suddenly appears and hands Tony a roll of toilet paper.
Pussy, meanwhile, bitterly turns over a micro cassette and his cut of the take over to Skip, who hands him back a couple of bills from the thick envelope.
The next morning, Artie Bucco comes to the Sopranos home to finalize the menu for Meadow's graduation party, but Tony accuses him of serving bad shellfish. Artie calls Pussy, who also ate the mussels, but he is feeling fine, so they think it must have been the Indian food that Tony also ate. A worried Carmela calls in Dr. Cusamano, while Tony dreams of having sex with Dr. Melfi on the desk in her office. One of Tony's last dreams features a fish speaking in Pussy's voice, stating what he knows, but doesn't want to hear. Pussy has flipped, plus he knows he's going to sleep with the fishes.
Even though Carmela admonishes him, Tony and Silvio head to the Bompensiero's house, under the guise of testing a boat, so they can find proof that Pussy's working with the feds. Faking another bout of vomiting, Tony finally finds the wire, hidden in the bottom of a cigar humidor. Pussy nervously looks over his shoulder when they meet Paulie at the marina and the four of them get on the boat. Under pressure from the others, Pussy confesses that he's been working with the FBI for a year and a half, but has mostly engaged in disinformation. Silvio, Tony and Paulie have no recourse but to open fire, and dump his body off the boat.
As if the day wasn't grueling enough, Tony is back at home when Livia calls from Newark Airport, where she and Aunt Quintina have been held by security all day. In a fit of rage, Tony had given her airline tickets from the Scatino bust-out and now it's coming back to bite him on the ass. He has barely hung up the phone when the FBI comes to take him into custody, right as Meadow returns home on the evening before her graduation.
Tony makes bail and is out of jail right away. In therapy, Dr. Melfi admits to Tony she was frightened of him, but he only responds with flippant comments. Later, at the graduation ceremony, Tony informs Christopher that he too, might be graduating soon.
