The Master highest on the list of Masters is the highest ranking master, same for Knights, but not Apprentices, they are all equal.
(1, 2, 3, and 4 are all Sith Lords, I am the Sith Lord)
In the event of Dark_Jedi being banned (or missing for an extended period of time) his hand will take his place, if his hand is banned as well then, the top Sith Master will take Dark_Jedi's place untill he (or his hand) are allowed to return. If all Sith Masters are banned then the top Sith Knight will take Dark_Jedi's place untill he returns, etc.
