How to keep your documentation ahead of Management

1) When calling in for a condition that is FMLA, you must tell the supervisor answering the call this at the time of the call in. Be sure to note who took your info at the call in. To qualify the condition should last for 3 days or more or require at least two visits the the Health Care Provider (Doctor), there are other qualifying standards, this is a quick rule of thumb.

2) When having the APWU Form 2 completed by the Health Care Provider (Doctor), keep the probable time and duration at 6 months or a year. Indefinite and permanent are not acceptable. Then make 10 copies of the form before you turn it into your supervisor.

3) Then when you have an absence due to the condition covered by the APWU Form 2 that was submitted, take one of those copies and staple it to the APWU Form 4 that you will fill out yourself and you sign it. Then submit these with a PS Form 3971 to cover the documented leave.

4) It is advisable to have the APWU Form 2 completed by your Health Care Provider (Doctor) upon return visits. This will advance the year duration window and help in staying ahead of Management.
