
Check to see if you have Microsoft Word installed:
1.) Open the form that your having problems with. 2.) Left click "help" on the top right of that window. 3.) Left click "about ____". (it will say about wordpad, or about word, etc.) 4.) A window will then pop up, if that window says anything else besides "Microsoft Word" then you need to get Microsoft word installed on your computer to fix the problem. If it DOES say Microsoft Word then there's something else wrong that needs more attention to solve, give the I.T. helpdesk a call for more assistance.
Your file is corrupt!! If you got this file via e-mail and you've had it resent and your still having the same problem then you need to obtain it by alternate means. If what you are looking for is not in the list I would recommend you try having the form sent to an alternate e-mail address (exp. yahoo, hotmail etc.)

Some people with laptops have seen this problem. Apparently there is a key on most laptops keyboards that toggles from numbers to letters (due to the fact that laptops generally don't have a number pad). You must have accidentally hit a button to turn on the numbers. What you need to do is find that button and hit it again. Since the name of the button varies from laptop to laptop I cant tell you what button to push, but what I can recommend you do is look at the key that your having problems with and see what color the number is, if its a different color then the letter then look for a key that's solely that color. If this isn't the case, then try using a different keyboard.
