ACES Workbook (Continued)
Typical, familiar old method of SOAP charting
You’re all pretty familiar with this kind of soap chart. All the wonderful stuff
with the client goals, aggravating, relieving, treatment goals, reduce headache
pain, pain in calves, reduce HT. That’s all nicely documented, but it still
doesn’t say much about the client’s ability to function and the affect on ADL’S.
Our goal is to show you how to:
Make your injury treatment SOAP charting nearly bulletproof
Change, reduce headache pain and reduce tension
in calves to function based treatment goals
Write treatment goals and treatment plans that use measurable data, even if
some of the data is subjective information.
Use the knowledge and tools you already have (physiological effects etc.)
as a rationale and justification for the strokes and techniques you use.
Justify what you did and why you did it
Tie this all together to save time and increase reimbursements. Insurance
companies or lawyers will have a hard time arguing when
you’ve got physical proof. There’s the function, I measured it, it is improved
The focus of this seminar workbook is on the initial intake and the
documentation process. However, since that is not the end of the
work, you will also find information on
sequential/supplemental
SOAP’s, status reports and narrative reports at the end of the lessons.
When you finish the workbook you will be able to write treatment goals
and plan like the ones shown below.
