I/my child agree to have positive sportsmanship at all times before, during and after the national event, and while representing said pageant system. I also agree that my family, friends and guests must also have positive sportsmanship or I/my child could be disqualified from said event.

I agree that the age division cut-off date is the date of the last scheduled preliminary pageant (March 18, 2001). I certify that all information submitted on the entry form is true, and agree to provide and submit proof of age, upon request.

I agree to hold harmless 5 Star Pageant Productions, Pacific Star National Pageant, the Sheraton Hotel, their said affiliates, and/or their staff from any loss due to accident, theft, damage, or injury.

I agree that the judges' decisions are final.

I/my child agree that if awarded a National Divisional title, I/he/she must make at least two documented appearances and attend the 2002 National Pageant in order to be able to receive the awarded savings bond. I/my child also agree that I/my child may recompete at the end of my reign, but only if moving up in age divisions. Grand Overalls may recompete two years proceeding their reign.

I/my child agree that if awarded a national overall hi-point title, I/he/she must make at least two documented appearances and attend the 2002 National Pageant. I also agree that I/my child will be awarded 50% of the cash award on stage at the 2001 National Pageant and the other 50% of the cash award will be presented at the end of my/my child's reign at the 2002 National Pageant.

I/my child agree that if awarded an overall hi-point title, I/my child is not eligible to win their age divisional title, however, I/my child can win the Super Star award and/or win or place in any of the optional events.

I agree that no cameras or videotaping will be allowed in the ballroom at any time, except during the pizza & dessert party. I will advise all guests, family, and friends of this rule. Failure to comply could result in confiscation of said equipment until the end of the pageant day.

I/my child agree that prizes awarded are considered full compensation for appearances requested, and that 5 Star Pageant Productions has permission to use my likeness for any advertising related to the event. I also agree that royalty members and families are responsible for any and all fees accrued while in attendance at any scheduled or non-scheduled events.

I agree that all deposits and fees are non-refundable, but can be transferred to the following years National Pageant.

I agree that 5 Star Pageant Productions and Pacific Star National Pageant reserve the right to substitute prizes/awards of equal or greater value.

I agree that failure to submit a program book picture by designated date will result in a point deduction from said contestant's total mandatory score.

I/my child agree that said contestant must compete in all mandatory and optional events as stated for age (excluding ad sales) to be eligible for an overall hi-point award.

I/my child agree that failure to comply with any of the National Rules and Regulations, or to show positive sportsmanship during my/my child's reign, if selected as a queen/king or overall grand, could result in the loss of any and all fees, titles and the forfeiture and return of all prizes, awards, and gifts.
