2010 50% off Enrollment Form

SINGLE MEMBERSHIP: $4,200.00
$2,100.00 will be applied to 2011 dues. Include $500.00 check with enrollment. Balance of the billing will be $1233.00 in each of the following three months. All dues are subject to NY State tax. All payments are non-refundable.
 
FAMILY MEMBERSHIP: $5,500.00
$2750.00 will be applied to 2011 dues. Include $500.00 check with enrollment. Balance of billing will be $1666.00 in each of the following three months. All dues are subject to NY State tax. All payments are non-refundable

Once you've sent us your information, please mail a check for $500 and indicate which membership type you would like (single or family). By sending in your check, you agree to the "Import Notes" listed below and to follow the Port Jervis Country Club rules and regulations, such as dress code and guest play. You may begin play immediately upon confirmation of receipt of your deposit (weather permitting). Details are below:
 
Important Notes
  • Family membership: Includes spouse, dependent children under the age of 25 who are full time students or active members of the armed forces, or life time same sex partner.
  • Grandchildren under 12 play free with either membership.
  • Offer includes shed parking for owned or leased golf cart. Attractive per play cart rental/leasing available.
  • Monthly minimum restaurant spending requirement , May through Oct., in the Jack Champion Steakhouse.
  • If a credit card is used for payments a 3% fee will be added.
  • Member pays approx. $20.00/year for Metropolitan Golf Assoc. membership and $9.00/year handicap fees.
