In Advanced Dungeons and Dragons, (AD+D) there are many realms and worlds. Although these realms are split apart and very different it is possible to travel to another realm. The realms that i have played in,
Forgotton Realms,

Ravenloft, and
Dark Sun. There are more worlds but i only have played in these.
