Choose an item with an effect on it already, so you can change the effect into something else.

Drop everything but this item, and put this expendable item in your free hand.

Save.

Edit the save in the following manner:
Set the '*Effect Act. Capable' to 134, for infinite item activations
Set the '*Effect # Act. Type' to 16, to define that effect as the one to be activated.

Set '*Item Effect #' to 124
124-When activated, summons a monster

Set '*Effect # Sum. Mons.' to one of the numbers in the list to be able to summon infinite numbers of that monster.

Reload the modified save.

Wear the equipment.

Use the Activate menu to summon the monster you chose as many times as you want.

Item Class, Item Subclass:

This has only been significantly tested by me in Castle of the Winds 2 saves, but everything on this list I've tried so far has worked in Castle of the Winds 1 saves, even the Helm of Storms, though it doesn't have space for all the special effects. If you really want to, you can get a weapon with 4 existing effects, modify the '*Item Class' and '*Item Subclass' to match the Helm of Storms, and modify it's attributes to your liking. It can then be an accurate Helm of Storms. Even in Castle of the Winds 1, the helm absorbs the charge of a rune of return.
