At the Western Terminus, NC 65 barely ends just inside Winston-Salem. Since NC 65 begins and ends on the south on/off ramp of US 52, it falls into Winston-Salem, as oppose to the north on/off ramp of US 52 which is in Rural Hall.

NC 65 connects the small communities in Forsyth and Stokes counties to Winston-Salem. From Walnut Cove to Reidsville, it's a quite secondary road, opposite to US 158.
