The above
named proposed member of Diabetics/Disabled Anonymous, Non Profit
Support Group and Alliances (herein after termed DDA)
has requested extensive benefits and services from the organization
which has no membership fee or minimum donation. Said member understands
that DDA is wholly self supporting by the contributions of the community,
fund raising through Celebrations of Healing and Knowing
and members donations and does not receive grant funding of
any type. All members and/or friends and/or family of the members
in the event the members cannot do the volunteer work, are required
to put into the organization what they take from it to keep the organization
healthy and strong, and to insure that all members contribute equally.
Only a decision by the full board of directors can allow an exception
and the above named must have proof of such an exception in writing
from the board or the president of DDA DDA is not a government agency
and does not operate as such.
The above named member, or friends and family of the member in the
event the member cannot do the work, contracts to do work services
for DDA, as a volunteer, for a number of work hours, proportional
to the value of the services received. This is not an equal exchange
of hour for hour but is based on the hourly value of the services
the above named member does and the hourly value of the services received
from DDA In the event that the above named member does not have services
that DDA wishes to use or cannot use, or does not want to do or do
these work services for DDA, the above named member will be liable
to pay DDA for the full commercial value of the services received,
with attorneys fees and costs; IMMEDIATELY UPON DEMAND. DDA reserves
the right to bill by a flat fee or itemized method, and the above
named herewith agrees to this. The above named does not obtain any
rights to monitor or control DDA by this agreement. Volunteer in this
context does not mean free or by whim or option
of above named member. One of DDAs goals is to promote member
self empowerment by work and volunteerism.
All work must be approved of in advance and by a written notation
by D.D.A; and scheduled through and by DDA The above named must keep
written records of their work hours and obtain the signature of the
president, Rev. Duchene, or other board member verify and approve
the work hours done , within three days of its completion, or
the hours will be discounted.
In the event that the above named refuses, twice, to provide services/work
hours that DDA requests; that will constitute a failure to comply
with this contract and the dollar value of the services provided will
be due immediately, inclusive attorneys fees and costs.
Suspension
or termination of membership can be employed as penalty for such refusals
and for member instigated or related (involving third parties connected
to the member) slander/libel, retaliation against this organization
for refusal to waive the work requirements, herein; or misconduct.
That regardless of work services, all members may, at DDA discretion,
additionally be liable to pay in legal tender for the cost of items
inclusive but not limited to: telephone calls at actual cost, faxes
at $1.50 per page outgoing and $2.50 per page incoming, photocopying
at 50 cents per page, word processing at $1.00 per page, computer
use at market rate, and other tangible costs and that these costs
will be payable within 30 days of use if credit is approved. The above
named agrees to DDA setting the rate for commercial values
mentioned herein.
The member agrees that DDA does not provide legal or medical advice
and is not in any way liable for volunteer assistance given or for
goods donated to members of DDA.
DDA staff
are all volunteer status and cannot be available at all times to the
above named. Abuse, slander and hostility of other DDA members, the
board or president is grounds for immediate suspension or termination
of membership, at DDAs discretion.
DDA is dedicated to the personal, social, economic, legal, spiritual
and political empowerment of persons with disabilities and the elderly.
Part of said empowerment is believed to be achieved by self sufficiency,
and working to help and cooperate with others who have similar difficulties;
which this pledge endorses and enables.
