By completing this e-mail form you are authorizing the information you send to be printed in the 40 Year Class Reunion Booklet on CD. To have your information included in the 40 Year Class reunion booklet on CD please compete the Reunion Booklet Form. ONLY e-mail responses will be included in the booklet.
Send a electronically labeled picture that you would like to have included in the booklet as an attachment and e-mail to edhs1969@aol.com before 10-10-09. You may also send a picture to EDHS 1969, P. O. Box 542, New Baltimore, MI 48047. Enclose a self-addressed stamped envelope if you would like your picture returned.

In addition to submitting the form below, write a paragraph about yourself. Be sure to include your educational background, current occupation, names and ages of your children, grandchildren, how many of the past class reunions you have attended, a strange funny, or amazing story about yourself or a classmate past or present and have you realized your dreams of the future!
