• Monday thru Friday 9 AM to 5 PM

• Tuesday evenings until 7 PM

• Saturday 12 PM to 5 PM

Professional Fees and Insurance

To help contain costs to all patients,

payment for office visits is appreciated

at the time of service.

Fees differ according to the treatment

and health care needs

of each individual patient. My office staff

is available to help you with insurance forms

should you require assistance.

Payment options include Visa, Mastercard,

checks and cash.

I participate in many PPO'S.
