Completing the Merit Badge Counselor Information Form:
Complete Information at the top of the sheet.

Under "List merit badge subjects here", list the merit badges to be counseled.
A complete list of current merit badges is listed on the back.  A check
indicates that Scouts need this badge in order to receive the rank of Eagle.
Insert an "X" under Vocation, Avocation or Special Training.  Include an
indication of qualifications on back of form.  Course in College is adequate
evidence of competence.

Under "Check One"
"I wish to work only with"  for Merit Badge Day, insert "Chem MB Day" or "Amer Labor
MB Day".  This will restrict your counseling activities to the specified Merit
Badge Day.
"I wish to work with all unit" will list you in the Patuxent District Merit
Badge Counselor Directory.

Under Current Position in Scouting, indicate "Merit Badge Counselor" if this
is your only Souting position.

If an adult application is attached, a signiture is not required.
