ABOARD CARNIVAL CRUISE LINES FANTASY (actual ship name will vary with your choice of scheduled
departures) ALL FOOD, ACCOMODATIONS, SHIPBOARD ACTIVITIES INCLUDED (cost of alcohol/casino gambling NOT included)
Winners will be responsible only for tips and gratutities they may wish to disburse, cost of alcoholic beverages,
cost of special activities (i.e. scuba diving, snorkeling, Bahama Island entertainment they may wish to participate
in that are not included in cruise costs)
WINNERS MAY ELECT TO TRANSFER PRIZE TO ANOTHER NAME, OR MAY ADD ADDITIONAL FAMILY MEMBERS(ie children) OR
UPGRADE AIRFARE/CABIN ACCOMODATIONS OR EXTEND THEIR FLORIDA STAY WITH DISNEY WORLD/ATTRACTION VISITS/FOR SMALL
ADDITIONAL COSTS AT THEIR DISCRETION. THIS AND OTHER CHOICES WILL BE OFFERED AT THE TIME WINNERS ARE
ARRANGING THEIR VACATION RESERVATIONS.
THE PURPOSE OF THIS CONTEST IS FOUR-FOLD IN GOALS AND FOCUS:
1.) TO PROVIDE A METHOD TO RAISE MONEY FOR LYME DISEASE RESEARCH AND HELP TO PROVIDE
LYME DISEASE TREATMENT ASSISTANCE FOR VICTIMS.
2.) TO PROVIDE AN OUTLET FOR LYME VICTIMS AND THEIR FAMILIES TO SHARE, IN WRITING, THEIR
EXPERIENCES STRENGTHS AND HOPES WITH OTHERS.
3.) TO COMPILE A COLLECTION OF WRITINGS BY LYME PATIENTS & FAMILIES FOR PUBLICATION IN BOOK FORM IN
NOVEMBER OF 1999.
WE WILL NOT INCLUDE ANY ESSAYS OR STORIES WITHOUT YOUR EXPRESSED WRITTEN CONSENT, (EVEN IF YOU WISH
TO HAVE YOUR STORY INCLUDED/PUBLISHED ANONYMOUSLY.
4.) TO PROVIDE A POSITIVE AND POWERFUL FORUM BY WHICH THE VOICES OF LYME PATIENTS AND THEIR FAMILIES
CAN BE HEARD, TO PROVIDE THE PUBLIC, THE GOVERNMENT, AND INSURANCE PROVIDERS A KEEN INSIGHT INTO THE
TROUBLES FACED BY VICTIMS OF THIS DISEASE.
TO ENTER CONTEST
CATAGORY #1 SUBMIT AN ESSAY (MAXIMUM 1000 WORDS) ON THE SUBJECT OF "WHY I NEED SHOULD WIN THIS DREAM VACATION OVER ALL OTHER CONTESTANTS" Include your name, address, and a self addressed stamped envelope along with an entry fee of $50.00 payable to J. J. Foley, Foley Real Estate, P.O. box 949, Belchertown, Ma. Make a notation on the check or money order designating it for the Lyme Essay contest, and be sure your name and address is on the check. All entry fees will be deposited into a contest escrow account and held until March 30th, 1999, or until a minimum of 150 entries have been received. Upon that date, or upon receipt of 150 entries, all entries shall be reviewed by a panel of judges, and a winner announced on or about April 30th, 1999........ Should contest interest levels result in less than 150 entries are received by the date of May 30th, 1999 all entry fees will be returned (thats the reason you must include a stamped, self addressed envelope with your entry)by the date of May 30th 1999. CATAGORY #2 Submit A Narrative story, not to exceed 5,000 words, dealing with your experience with Lyme Disease. Entry may include any positive/negative experiences with doctors, hospitals, insurance providers, etc., but we reserve the right to edit any proper names included prior to publication. Entries will be judged according to content, writing style, grammar,punctuation. In the event of a tie, and should the judges be unable to decide the winning entry in either the essay or story division, all winning entries will be placed together, and a grand prize winner be chosen at random. Each entry must be accompanied by a $50.00 check or money order, payable to J.J. Foley, PO Box 949 Belchertown Ma. 01007. As in catagory #1, should less than the minimum necessary number of 150 story enteries be received, all entry fees wqill be returned by May 30th, 1999. No more than 300 entries in each division will be accepted. All entries in excess of 300 in each division will be returned to the submitters. Judging and prize awarding will commence upon receipt of the 300th entry in each catagory, and prizes may be awarded prior to published deadlines.
