Character's family background: In this box, write breifly on your character's life was like before joining Starfleet, especially involving his/her family.
Physical/psychological background: Summarize your character's build, any special physical or mental abilities, and a breif psychological profile. Do not make your character immortal, able to fly, etc. because that sort of thing can get in the way of a storyline.
Starfleet record: Summarize your character's previous Starfleet positions. Since he/she probably just left Starfleet Academy, you can simply summarize his/her performance there, possibly including field postings during training.
