5. Your e-mail address (required so I can send you a confirmation)
6. Payment Method (PayPal, Check, or Money Order)
7. For each item you would like to purchase please list the item number, item name, quantity, and price each.
8. Any questions or comments you may have.
I will be receiving your order the next time I check my e-mail.
