By clicking "TAKE SURVEY" I give the author of survey, "How Did You Do It" (working title of what is henceforth referred to as the Property), the absolute right and permission to copyright and/or publish or to have copyrighted or have published information about me contained in the above mentioned Property. I understand that a pseudonym can be used in place of my real name and that other physical attributes can be fictionalized in order to protect my right to privacy. I also hereby waive any right to inspect and/or approve not only the final edited-for-publication versions of the book, but also any advertising copy that my be used in connection therewith. I also agree that the author or the publisher shall have all the irrevocable and unconditional dramatic and subsidiary rights to the Property and can cause an actor to depict actions in incidents related to, based upon, or adapted from me as depicted by the "character" based upon me in the above mentioned Property. Further, such adaptations may include fictional scenes, incidents or episodes based upon but not specifically included in the Property in preparation or production of a picture, and I waive all rights to the exploitation of such a picture in all media throughout the world, and in connection with customary advertising and publicity related there to. I represent that I will not assert or institute any claim or action against the author or the publisher or other purchaser of rights based on the Property, on the grounds that their use of the above described rights violates my right of privacy or any other right belonging to me. Nothing contained herein shall be construed as prejudicial to any rights to which I may otherwise be entitled as a member of the public. None of the warranties or indemnities I make pursuant to this Release or else where herein shall extend to (a) any changes or additions made in the Property, or (b) any elements of the Property which are undertaken to fictionalize for dramatic purposes. I agree to indemnify and hold harmless the author, publisher, or subsidiary rights' purchasers from and again any claims arising from such Charges.
