Organization	Observation is well organized and easy to understand.	Organization and format are confusing at points, but clear overall.	Organization and format are confusing.	Observation is not clearly organized or formatted.
Content	Observation has all needed information.	Observation has adequate information.	Observation does not have all needed information.	Observation is lacking useful information.
Bias	Language is free from bias and judgment of behavior.	Language is free from bias and judgment with 1-2 exceptions.	Language is free from bias and judgment with 3-5 exceptions.	Language has a large amount of bias and judgment.
