Provide a description of steps your faculty will

1.
Download a copy of the outcomes map template.
2.
List the program learning outcomes on the vertical
axis and courses that you teach horizontally across the top.
3.
Identify courses
that support each learning outcome and assignments that may be used for
assessment.
Create and Implement an Assessment Plan
1.
List at least two program learning outcomes.
2.
Explain how you will determine if outcomes are
achieved (assessment procedures and performance criteria).
3.
Conduct assessment activities, analyze results and
report findings.
4.
Use findings to improve
teaching methods, course materials and activities, assessment procedures, etc.
