This letter constitutes my formal request pursuant to the Texas Public Information Act § 552.001, for public records concerning the above referenced doctor, who claims to have passed all requisite medical examinations for the purpose of practicing medicine in Texas.
I request that your agency conduct a complete and thorough search of all systems of records, files, data bases, and other information sources maintained by your agency and/or on behalf of your agency, and provide me with usable copies of all personal and professional information about Dr. Lisa K. Clayton.
More specifically, I want you to list current and previous license information. Please list for me the dates of licensing; if applicable, when issued, suspended, revolked, restricted, and/or placed on temporary status and the reason for the action.
I want to know when this doctor received their licenses from this board or the state, the license numbers, the expiration dates and/or the renewal date of said licenses.
I want to know the doctor's educational background from grades 1 through college and post graduate.
I want to know what specialties this doctor is certified in and the name and address of the accrediting authority. Also provide the doctor's full name, date of birth, business address, list also in the records provided all medical licenses this doctor has to practice medicine inside and outside of Texas.
List all malpractice claims filed against her and/or that were paid out. List all complaints/grievances filed against this doctor, dating back to 1990.
If any information sought is denied in part or in whole, please specify the exemption being claimed for each passage and how that exemption is believed to apply to the information I requested.
As a representative of Making The Walls Transparent, a non-profit organization that disseminates information to the public, the information sought is considered under the "compelling need" clause and I am seeking a waiver of fees after the first 100 pages to which I am initially entitled.
Should you foresee any responsible delays, please inform me of this and the date your agency will be able to act on this request. Otherwise, I shall expect my request to be fulfilled within ten (10) business days from today's date.
