INSTRUCTIONS AND DISPOSITION:

Activity - As tolerated.

Medications - .

Compazine 10 mg p.o. q.6h p.r.n. for nausea and vomiting. Nicotine patch 21 mcg per day.

Allopurinol 300 mg p.o. every day.

Prozac 20 mg p.o. everyday.

Vicodin 5/500 two tablets p.o. q4h p.r.n. for pain.

Seroquel 300 mg p.o. q.h.s. 150 mg q.a.m. and 50 mg p.o. q.6h.
