B6-4
SM Park, east site Major operator error: Left the safety cap (with the Estes key) on the rod then used the Aerotech controller. The rocket took the top section of the launch rod with it. Got stuck in a tree. Three days later the body was recovered after a recent rain softened the glue on the shock cord mount. Update: The original nose cone and what remained of the streamer were recovered on the ground on 2001/04/01 (no foolin'!), 306 days after the launch. The streamers were torn and faded badly and the shock cord had lost nearly all of its elasticity, but the nose cone (and paint) were in great condition.
