Application for Companion Dog

The application information enables us to determine the best temperament and personality match for you/your family after our temperament testing which is done at seven weeks. It also provides us with information to determine if you are going to be an appropriate match for one of our puppies. Baycliff puppies are placed in approved homes. This form must be complete. Incomplete forms, and forms without references will not be considered. We will gladly provide references for you as well.
