Instructions for filling out FRIENDS Grant Application Form

Applications must be submitted in writing, using the Friends of North Texas Lacrosse Grant Application Form ONLY; form must be typed or hand printed and should include all required documentation.  Incomplete application forms will be deemed ineligible until they are completed.  We will try to turn around these applications in the shortest time possible but in any event, applications will be approved, requested to be modified, or denied no more than 90 days from the date of receipt of the completed Application Form

Going through the Application Form line by line:

Name of organization, date, address, city, zip code, telephone number, fax number, email are self-explanatory

Contact individual:

The person who can answer any and all questions regarding this application, including that individual’s position within the organization.  This must be someone who is representing the organization in an official capacity.  Include contact information for this individual

Organizational status:

Check box(es) defining how your organization is structured.

Membership and Additional Information

What defines membership in your organization? (i.e. attends xyz school or church; lives in xyz area)

Indicate whether this is an ongoing organization or an administration created for a one-time activity or event

Affiliations

Is your organization affiliated with a local school, sponsoring organization or a local, state or national alliance; check box corresponding to the type of insurance policy the organization has

          Mission Statement

Include your organization’s mission statement, or, if it doesn’t have one, describe the organization’s range of activities and its goals. 

Current method of funding:

This information will significantly impact your organization's credibility.

Include specifics on your organization’s financial support, if any, from memberships, donations, parent‑groups, school or community funding, etc.

Description of Your Proposed Project

Check off: Startup Program; One‑time project/event; Ongoing/Annual project/event.  Define the organization, activity or event for which you are seeking support.  Include a short description of project, including its goals, and who will benefit from the project.

Time Frame for Project’s Completion

Include a timetable for completion of project/event/activity

Request for Resources

Check off Sponsorship/Administrative support and/or Grant in the amount of $____

-If sponsorship/Administrative support is checked, describe the specific contractual arrangement you seek.

-If grant money is requested, indicate the specific amount you are asking for and check off either the Monitored grant or Repayment proposal option. 

                 -If Repayment option is checked, include a timetable describing when you will

                    repay the grant

Required Documentation: (Copies of documents must not exceed an 8 1/2 " by 11" format)

              Budget Projection: If the application is for sponsorship/administrative support or funding a portion of an activity, a partial budget will be sufficient. If the request is for sponsorship/administrative support of an entire event, a comprehensive budget is required.

Additional required documentation:

Organization’s history: a brief summary of how, where and when this organization was founded, any past publicity, critical reviews, and/or letters of support (no more than five pages), insurance documentation, if any, including a copy of your policy.

Additional Application Information

To strengthen your proposal, it’s helpful to include the following in your application: 

For "Start‑up" programs, proposed events or organizations, a list of active participants, including the names of key personnel, sponsors and affiliations. This information is helpful in determining if the project has a solid foundation, or is still struggling to get organized.

Also helpful is to document the amount of parent and/or community support, whether or not you have trained (or untrained) coaches, a roster of your players, minimal safety equipment (first aid kits, parent or coach with any sports medicine or medical training available to the team), basic uniforms, a practice schedule  and an available practice/game field(s).

Startup packages will vary in content depending on the needs of the requesting team or organization but will always include game basics such as goal cages with nets, lacrosse balls, time clocks, scorer’s table etc.

FRIENDS will retain ownership of Startup packages for a period of two years.  Within that two-year period, should the program achieve basic standards, the Startup package will become a permanent grant.

If you are seeking funding for a specific activity or event, include an estimate of how many people will

participate in this project, including the names of key personnel, the advantages supplied by the facilities where the project will take place, and follow-up activities.  Detail public access, parking, how the event will be promoted, including an estimate of the racial, ethnic and/or gender participation and any special constituencies served by the project.

Should your project require shared responsibility in administration or sponsorship, please provide a concise summary of exactly what you expect of FRIENDS.

If your project requests a financial grant, please detail states of completion for purposes of monitoring the project at the option of FRIENDS or a contractual arrangement to return all, or a portion of, the funding.

In attaching your budget projection for a Grant application or related project, make sure to include estimated expenses and estimated revenues.

Your submission of the completed application form indicates that the entire application and instructions have been read and the request for all information contained in it is acknowledged.  In the event an application is rejected, we want to know that the applicant understood what their obligations and responsibilities to provide the requested information were.
