This is a one page summary of benefits that are condensed from the pages in the booklets.
This plan has a benefit for the family even if only one person in the family is insured for the health insurance. The accident benefit pays up to $2,000.00 for accidental medical expense for each accident, and could waive the deductible for the health benefits. If a person has an accident during a calendar year, and the medical expenses run up to the amount of the deductible; the deductible has been satisfied.
Another benefit pays up to $500.00 (before the deductible) for an emergency that is not an accident, such as a heart attack or appendectomy,...etc. Most people never have to use their deductible. Doctor's visit co-pay is $15. and is unlimited for the visit. Most companies have a $50.00 limit for the doctor's visit.
Each person is insured for $3,000,000.00 maximum medical expense. You have a choice of deductibles from $500. to $1500. per calendar year. The company pays 90% of the remaining charges up to $10,000. and 100% of all charges over $10,000. The most out of pocket expense you would have to pay would be $1,000.00 in addition to your chosen deductible. There is no deductible for doctor visits or preventive care charges. You only pay a $15.00 co-pay per visit. You will have a prescription card that can save more than the normal prescription co-pay.

daily room and board up to the semi-private room rate
intensive care charges - no limit
inpatient miscellaneous services and supplies
ambulance transportation, by land or air --- unlimited as needed
primary surgeon
an assistant surgeon, if needed
an anesthesiologist or anesthetist
a radiologist
a pathologist
a second surgical opinion up to $250.00
emergency services up to $500 not subject to the deductible for sickness and up to $2,500. for accident with no deductible.
home health care for up to 120 days
hospice care for up to 6 continuous months
annual mammograms and prostrate exams have no deductibles
medical equipment rental or purchase if needed.
