GOVERNMENT HEADSTONE OR MARKER FOR YOUR CIVIL WAR ANCESTOR

The U.S. Department of Veteran Affairs (VA) furnishes upon request, at no charge to the applicant, a Government headstone or marker to mark the unmarked grave of an eligilbe veteran to any cemetery around the world.

When burial is in a private cemetery, an application form 40-1330 must be submitted by the next of kin, funeral directory or cemetery representative.

Forms can be obtained at any VA regional office or by calling 1-800-827-1000. All completed appliations should be mailed to: Memorial Programs Service (403), Department of Veterans Affairs, 810 Vermont Avenue, NW, Washington, DC 20420-0001

Forms can be obtained from the Hawkins County Veterans Service Office at the Courthouse in Rogersville, Tennnessee.
