Sailor Moon has 3 forms. Sailor Moon, Super Sailor Moon, and Eternal Sailor Moon. Her original form, Sailor Moon, was brought up by Serena meeting Luna and using her imperial crystal to transform. I don't know too much about Super Sailor Moon.. but her outfit was a little differnt ^_^. Then there's Eternal Sailor Moon, who got her wings from Helios (the priest of Elysion) and of course defeats Galaxia, with Chibi-Chibi in her sword form, and takes Sailor Galaxia out of Chaos's control.
