Dear Parents,

I am excited about having your student in my class this year.  I try to make my classes informative, fun, and a comfortable place to learn.  To make my classes more effective, I need your help.  You know your student better than anyone else and I am asking that you share some of your information with me by answering the following questions about your student and their study habits.  Please complete this survey and return it on Friday.  This information will help me design a learning program that is centered around the needs and strengths of your student.  If you ever have any questions or concerns, please feel free to call me at 949-2355 ext. 352 or email me at  Thank you for your help and support!
