to obtain Pre-Participation Physical Forms on-line.
YOU MUST DOWNLOAD,PRINT, and COMPLETE ALL FORMS BEFORE SUBMITTING THEM TO THE SEAHAWK SPORTS MEDICINE DEPARTMENT.

Following is a brief description of the forms you will find here:
PHYSICAL DIRECTIONS FORM:
Explains the various forms needed for your physical and how to complete them.
PARENTAL CONSENT & WAIVER FORMS (2 Pages):
Provides consent for travel, medical evaluation and treatment, and
explains the risks involved in participating in interscholastic athletics.
HIPAA MEDICAL RELEASE FORMS:
Federal law that went in to effect during 2003 require the Horry
County Schools to have the consent of parents before medical
information about their child may be released to or exchanged with
physicians, hospitals, insurance companies, and other health care
related entities. Parents should complete the release form and
return it with the other physical forms. Parents may keep the "HIPAA
Notice of Privacy Practices" for their records.
PREPARTICIPATION HEALTH SCRENNING FORM (2 Pages):
Parents should complete the first page in its entirety. Please DO NOT
LEAVE BLANK SPACES! All information is vitally important.
The second page of this form is to be completed by a South Carolina
licensed Medical Doctor, Doctor of Osteopathy, or a Nurse Practioner
or Physician Assistant under the supervision of a licensed MD or DO.
Exams completed by Chiropractors, or any other health care provider,
are NOT VALID under ANY circumstances.
NOTE...
Physicals are valid from April 1st of the current year to April 1st of the
following year. (For example, a physical given on April 1, 2006 is
good until April 1, 2007.)
