Disclaimer: The National Physician and Family Referral Project (NPFR) is a community outreach project which focuses on pharmaceutical and biogenetic research. In partnership with 50 Hoops, the National Human Genome Center at Howard University and other National and International Human Genome projects; NPFR conducts physician, community and public media surveys and research in locating African American families who meet the criteria for scientific human genome research in prostate cancer. The information we gather is voluntary and confidential, in that it will not be used for public or media dissemination. This information will be used for scientific and medical research. Names and identities of family members or participants will not be used in these non-invasive scientific research projects.

you are voluntarily offering important and historic data to human genome research and that no pay is/has been promised now or in the future for your participation in this phase of the project. You agree to hold harmless 50 Hoops and the National Physician & Family Referral project, its partners, associates, clients, employees, affiliates and consultants from and against all claims arising from use of this information for human genome research or other medical, clinical studies for which the physician or patient may be contacted or for personal or medical information release.
