The time of menopause

As hormone levels fall, a woman's pattern of menstrual bleeding usually becomes irregular. Many women experience light, skipped or late periods for several months to a year before their periods stop altogether. Some women may experience heavier-than-normal bleeding. It is important to realize that until menopause is complete, a woman still can become pregnant even when periods are light or missed.

Before menopause, women have lower rates of heart attack and stroke than men. After menopause, however, the rate of heart disease in women continues to rise and equals that of men after age 65.

Because women can still become pregnant while they are perimenopausal, doctors may do a pregnancy test when a woman's periods become irregular, infrequent or light. In some cases, a blood test for levels of follicle-stimulating hormone (FSH) may be recommended. FSH levels are normally high in menopause, so high FSH levels can help to confirm that a woman is in menopause.

At the time of menopause, doctors often recommend a bone density measurement. The test result sometimes will detect early osteoporosis. More often the result is used as a baseline to compare rate of bone loss in the future.

Estrogen taken as a pill or applied to the skin as a patch can reduce hot flashes, sleep disturbances, mood changes and vaginal dryness. Estrogen can be prescribed alone when a woman no longer has her uterus. A combination of estrogen and progesterone is used when a woman still has her uterus. Progesterone is necessary to balance estrogen's effect on the uterus and prevent changes that can lead to uterine cancer.

The use of soy products in the diet such as tofu may have benefit for some women. Soy has small amounts of phytoestrogens (plant estrogens) that may help relieve hot flashes. Researchers speculate that the soy-based diet of Japanese women plays a role in preventing hot flashes. However, it's not clear whether Japanese women have fewer hot flashes or whether they report this problem less often.

Sexual dysfunction or sexual malfunction is difficulty during any stage of the sexual act (which includes desire, arousal, orgasm, and resolution) that prevents the individual or couple from enjoying sexual activity.

Sexual desire disorders or decreased libido can be caused by a decrease in normal estrogen (in women) or testosterone (in both men and women) production. Other causes may be aging, fatigue, pregnancy, medications (such as the SSRIs) or psychiatric conditions, such as depression and anxiety.

There may be medical causes to these disorders, such as decreased blood flow or lack of vaginal lubrication. Chronic disease can also contribute, as well as the nature of the relationship between the partners. As the success of sildenafil (Viagra) attests, most erectile disorders in men are primarily physical, not psychological conditions.

The problem of an inadequate erection is probably one of the biggest issues a man confronts. Most men experience erectile problems on occasion, but impotence, also called erectile dysfunction, is defined as "the persistent failure to develop and maintain erections of sufficient rigidity for penetrative sexual intercourse." Of course, men have other kinds of sexual problems including lack of desire and problems with ejaculation, yet impotence is the most common and troubling.

An erection occurs when the nervous system activates a rapid increase in blood flow. The vascular muscle in the spongy area becomes engorged with blood and the outflow of blood is cut off. An erection can occur as a reflex as we see in spinal cord patients, or can be caused by psychogenic (originating in the mind) stimulation. Numerous sexual stimuli are processed by the brain and transmitted to the penis via the nervous system.

There are many effective treatments for erectile dysfunction. The most popular option is a class of drugs called phosphodiesterase type 5 (PDE5) inhibitors, which includes sildenafil (Viagra), vardenafil (Levitra) and tadalafil (Cialis). These drugs, taken in pill form from zero to 60 minutes before sexual activity, work in approximately 70 percent of men, though they are less effective in men with neurological causes of erectile dysfunction such as nerve damage from prostate surgery, diabetes or spinal cord injury.

Hoodia pills are my life saver. I have a thyroid problem and this causes depression and weight gain. These pills take my hunger away so I can get back into shape and they make me happy, nice "side effect" ;-) Now my Doctor is taking them too! M S (Germany).

The Hoodia Gordonii plant originated in the southwestern part of Africa and grows best in the semi-arid areas of South Africa primarily the Kalahari. In its native habitat it is extremely difficulty to identify because it looks similar to succulents of the same family of which there are many. Only an experienced botanist or as the case in South Africa with Hoodia Gordonii a San people can identify the plant prior to it's blooming.
