Teen Participation/Membership Agreement for Nature Spirit Community

who’s birthday is _____________ grant permission for him/her to have membership and/or participate in any of the activities and functions sponsored by the Nature Spirit Community Church Knoxville. I accept all legal and financial responsibility for their actions while at such functions and relieve NSC Knoxville and it’s members of any and all responsibility for any actions of my son/daughter/ward. I also understand that there is a NO TOLERANCE DRUG AND ALCOHOL POLICY, and hereby agree on their behalf to abide by said policy and understand that any actions in violation of this NO TOLERANCE DRUG AND ALCOHOL POLICY will be addresses in the manner set forth by the laws of the State of Tennessee.
