To
proceed with obtaining a new passport through Zierer Visa Service, please submit
the following documents to an official Witnessing Clerk (This clerk can be an
authorized representative located at a United States Post Office or the County
Court Clerk. Not all U.S. post offices are familiar with this procedure so
please speak to your local branch before proceeding.)

Proof of U.S. citizenship (one of the following documents)

Two (2) identical passport-type photographs
Photographs
must be official passport-type only and approximately 2" x 2" with a
clear background. Photos must be sufficiently recent to be a good likeness and
need not be signed. Glossy or matte finish photographs are acceptable. Machine
or home photographs are not acceptable. Do not wear a hat or any item that
covers the face (except prescription glasses). Face forward only; no side shots
or profiles will be accepted.
4)
Passport Application Form (Form DSP-11)
This
form is commonly referred to as the "brown form". Complete this form
in advance but do NOT sign the form. (Form is included with download.)
Please
keep a copy of your Passport Application Form outside of the envelope to send to
ZVS.
5)
Payment of US$145.00
The
witness sealing your documents may require a portion of the money paid directly
to their office as a processing fee, therefore bring two (2) checks.
6)
Letter of Authorization
This
letter will authorize Zierer Visa Service to submit and obtain the new passport
on your behalf. (Form is included with download.)
7)
Proof of Departure
Airline
ticket or an original itinerary will be needed. (Proof of departure will vary
according to the U.S. Passport Office issuing your passport.)
As
stated, please present all above listed items to the authorized representative
for approval. You will then sign the passport application form in the presence
of the representative who will witness your signature and select the following
items from the above list to place in an envelope:

The East of England
The East of England region is founded on the history of the olden empire
of East
Anglia and, or course, is accessible to and from London. Two major airports
(London Luton and London Stansted) are also located here, as is the
beautiful port of Harwich.
