REGISTRATION FOR FALL 2005 INTRAMURAL SEASON

I/We the parents of the named child, understand that in participating in a sport there is some risk involved. Therefore, by signing this registration authorization, I recognize that this element of risk exists and agree that in the event of injury to my child, the Glen Cove Junior Soccer League shall bear no liability except in case of gross negligence. I give permission to Glen Cove Junior Soccer League to use images of my child for league promotional purposes.
